Deadline for COVID-19 testing consent forms – Sept. 8

As the University prepares to welcome residential and commuter students back to campus on September 29, onboard and periodic COVID-19 testing for all students who will have an on-campus presence in the fall semester is required. 

These students will need to complete and sign two online consent forms by Tuesday, Sept. 8, 2020.   

Log into your myWestfield account to follow these steps to provide your electronic signature on two forms: 

Form 1: Consent to SARS-CoV-2 Testing and Waiver 

Form 2: Consent and Waiver of Liability  

  1. Click the “Quick Links” drop-down menu, and click “MyHousing.”    
  2. Click the “Applications” tab, and select “Form 1: Consent to SARS-CoV-2 Testing and Waiver”  
  3. Review the form and electronically sign at the bottom of the screen; click “Continue.”    
  4. Enter the Parent/Guardian information; click “Submit.”    
  5.  A confirmation screen will be displayed, and a confirmation email will be sent to the student’s email account. 

Repeat Steps 1-5 for “Form 2: Consent and Waiver of Liability”    

Testing will take place in the Woodward Center. The overall schedule for onboard and asymptomatic testing is as follows: 

Onboard testing: Sept. 24-27, 2020 
October asymptomatic testing: Oct. 13-16, 2020 
November asymptomatic testing: Nov. 2-5, 2020 
Nov./Dec. asymptomatic testing: Nov. 30-Dec. 3, 2020
